Inconsistency on the position of the outside air dampers (OADs) from one unit to the other. For example, AHU-5, 6 and 7 - OADs appeared to be 100% open while AHU-9 - OADs less open. Additionally, at the time of inspection it was 29 degrees outside temperature so I am not sure if the OADs should be 100% open.

AHU-6 and AHU-10 - Return fan not running while the supply fan and rest of the units were operational. Informed the HVAC maintenance department about this by email the same day. Inconsistency on the position of the inlet vanes from being 100% opened to partially opened to completely closed. For example AHU-9-inlet vanes completely closed.

Chiller #1 controls screen display message shows that it has been shut down and is in a alarm. Maintenance Department is suppose to get chillers failures alarms on their phones and/or computers whenever that occurs and can also check the alarm log and/or trends to see exactly when it went down. Don't know how long it has been down. Notified the HVAC department of this issue by email and attached a video to it showing the shut down message on the display screen.

Cooling tower was operational but appeared extremely dirty. Not sure if the required chemical treatment system is present and working and if the tower is being cleaned as part of the required regular preventive maintenance. Notified the HVAC department of this issue by email and attached the pictures to it showing the algae growth on the cooling tower. Also, the water from the overflow drain pipe is dripping and the pipe appears to be clogged and the water laying around it on the roof. Other drain pipes appear to be clogged as well.

Controls panel covers for units AHU-5, AHU-6, and AHU-8 were wide open and not locked. Closed them while on site but was unable to lock them. They should always be locked so no unauthorized person can access them.

Air handling unit (AHU)-5, AHU-6 and AHU-12- Flex duct connecter inside the unit between the supply fan and the duct ripped and has large hole causing a lot of air leakage. This is a repeat finding and has been reported before as part of previous inspections. This affects the performance of the units considerably related to total airflow, waste of energy etc.

A lot of brand new spare filters are stored in the mechanical penthouse and quite a few are laying outside the boxes in the open collecting dust and exposed to the outside elements, degrading filter quality.
