Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

October 12, 2022
The Office of Environment observed some mold growth on metal roof components in the auxiliary gymnasium during the school's IEQ general assessment. The growth is believed to be from past thermal differences between surface temperatures and ambient air conditions that caused condensation.
On October 14, 2022
Building Maintenance roofers inspected the interior and exterior of the auxiliary gym. The Office of Environment was contacted and reviewed area with the roofers. Signs of past water (condensate) droplets were observed on the top framing of sound panels. Moderate dust accumulation was observed on elevated horizontal surfaces (duct work, light fixtures, flanges of roof trusses, etc.).
Before scheduling remedial work, the Office of Environment reviewed past Heating Ventilation and Air Conditioning (HVAC) work orders and contacted the HVAC Shop as to the current status of HVAC operation of the main and auxiliary gym HVAC units. The main gym had a controller issue while the auxiliary gym has a failed condenser fan and compressor failure. Resolutions were implemented until repairs can be made before next cooling season. Building Maintenance indicated mold and cleaning remedial work could be scheduled.
November 3, 2022
The Office of Environment met with a board approved restoration contractor to review scope of work on November 3. Work was scheduled for November 22 and 23. The scope of work included cleaning/disinfecting mold areas then encapsulating with paint, cleaning horizontal surfaces of dust as a precaution, and sealing potential air gaps associated with roof trusses.
November 30, 2022
The Office of Environment reviewed finished work on November 30 and noted a section missed on the metal roof deck tucked in between the block wall and a roof truss.
December 2, 2022
The contractor resolved the missed area on December 1. The Office of th Environment assessed the contractors work on December 2. The work was acceptable. No further action is required at this time.
Building Maintenance was to review the exterior structure for potential sources for the mold growth on portions of the roof decking.
