Room 143 - The upper portion of the block wall (an exterior wall) near the teacher's desk has holes allowing possible odors to channel into the classroom (Work order 187618).
Portable 29 - The middle metal transition plate is unsecured in one section and floor tile becoming loose surrounding that section. Tripping concern along with possible void in floor leading to crawlspace beneath (Work order 187614).
Exterior of building between rooms 170 and 173 there is a void under a corner of the concrete padding around base of the school. Work order 187634.

Room 127 - Rust and water staining on a ceiling tile containing a sprinkler head. A sprinkler pipe connection showed signs of leaching debris on backside of the ceiling tile which permeated through to the exposed side of the ceiling tile (Work order 187617).

Throughout the school there are unlabeled spray bottles containing a blue liquid. Assumed they are there for the purpose of COVID. The bottles are to be labeled with manufacturer label (assuming a custodial product) to properly and readily identify the contents per the OSHA Hazardous Communication Standard. School custodial staff to acquire necessary labels and have school staff apply to bottles.
