Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Relative humidity mold growth was observed on various chairs throughout the classroom.
The HVAC department was contacted and indicated that the supply fan for the Heat Recovery Unit (HRU) servicing the area was not operating properly.
The remainder of the classrooms in the zone were assessed. No additional mold growth was identified.
Room 108 is an interior room with no windows and may be more susceptible to summer mold growth than the other rooms in the zone.
Building Maintenance (HVAC) reset the ERU which had tripped (off since August 24, 2020). Building Maintenance also adjusted some of the operating set-points to provide better dehumidification. Work on the ERU was confirmed complete on September 15, 2020.
A board-approved restoration contractor cleaned/disinfected chairs, desks, cabinets, tables, etc. An air scrubber equipped with a High Efficiency Particulate Air (HEPA) filter was operated in the room during the cleaning. The Office of the Environment re-assessed the space after the cleaning, no visible mold growth was identified.
