Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Historical Information
The Office of the Environment responded to a flood that impacted Room 127 on May 17, 2019. Response actions included utilizing a board-approved restoration contractor to remove impacted drywall and insulation. Fans and dehumidifiers were placed in the impacted areas to facilitate drying. The carpet was then cleaned by the restoration contractor. The response actions above were completed to prevent microbial growth.
July 8th Assessment
No mold growth or odors were observed.
No elevated moisture was detected in the carpet.
Temperature and relative humidity readings were acceptable (assessment occurred after school had let out for the summer).
Areas above the drop ceiling were inspected and were acceptable.
Fireproofing was observed above the drop-ceiling and is intact (no concerns).
The Heating Ventilation and Air Conditioning Unit (HVAC) that serves the space (AHU-5) was inspected. The filters inside the unit had collapsed. No other concerns were identified with the unit.
Building Services will assess/address the cause of the collapsed air filters and replace them.
