Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The leak appeared to have occurred due to a malfunctioning diaphragm and flush valve associated with the boys bathroom causing water from the supply line to continuously run and leak onto the ceiling outside the media center (below).
Plumbing replaced the malfunctioning components listed above and contacted the carpentry and electrical departments to remove lights from ceiling and assess the drywall ceiling.
Building maintenance placed fans within the area to dry water impacted materials.
March 14, 2024
The Office of the Environment visited the site to assess the materials. Several areas of drywall (less than 1 square foot of total area) were still moisture impacted. The Office of the Environment adjusted the fan to concentrate air flow as needed.
March 15, 2024
Moisture content within the drywall was still elevated in two areas along the flood light. The blower fans were further adjusted to concentrate drying efforts on materials still impacted.
March 18, 2023
No elevated moisture was detected.
The following response actions were completed to reduce the potential for microbial growth.
Building maintenance to remove blower fan and repair ceiling.
