Concern
Custodian reported suspect mold on the carpet in Room 1.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date(s) of Assessment: July 21 and August 8, 2025

Visible mold growth (approximately two-square feet) was confirmed on the carpet in the space. 

The relative humidity in the space was slightly elevated so the Office of the Environment recommended Building Maintenance assess the Heating Ventilation and Air Conditioning equipment that serves the zone (RTU-14).

Building Maintenance found that RTU-14 was not supplying the proper discharge temperature (too high). Building Maintenance reported the issue was corrected on July 29, 2025. 

The mold growth likely occurred due to carpet cleaning activities (wet) coinciding with an HVAC deficiency. 

It should also be noted that the same HVAC deficiency was noted for RTU-12 and 13. The Office of the Environment performed a precautionary check of these HVAC zones as well. No mold growth was identifed.

Corrective Actions
July 21st, 2025

Custodial Services was requested to extract/disinfect the impacted carpet, after confirmation that the HVAC system was operating properly. 

July 29th, 2025

Building Maintenance addressed the HVAC units with a high discharge temperature.
