IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: June 18, 2026

The weight room was observed to consist of bare concrete floors with interlocking rubber tiles, concrete masonry unit (CMU) walls, and drop ceiling tiles. 

Rust was observed on the door frame and moisture was identified underneath rubber tiles near doorway. 

Walls were observed using a thermal infrared camera. No thermal anomalies indicative of an active water intrusion were identified. 

Visible dust deposits and general uncleanliness was observed.

One square foot of visible mold growth on concrete wall underneath a paper towel dispenser within the weight room.  

Although a definitive cause was not identified, the mold growth observed on the concrete wall is likely related to moisture introduced by individuals using the adjacent paper towel dispenser. Water dripping from wet hands during use may be contacting the wall surface on a repeated basis, creating conditions conducive to mold growth. 

Corrective Actions
June 18th, 2026

Visible mold growth was cleaned from wall using a disinfectant. 

July 8th, 2026

The door sweep was found to be deteriorated and threshold was found to be never sealed. The threshold was sealed and a new door sweep was installed. 

July 20th, 2026

Custodial area manager was informed of deposits of dust present within the room. Custodial area manager is to work with custodial staff to develop a cleaning plan for the area.
