Concern
Strong wet moldy odor, active moisture and leak, and visible mold in wood cabinet.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment(s): January 7, 2019

Water staining from an apparent faucet leak was observed underneath the cabinet (Lab Station 9).

The leak was repaired by Building Services (Plumbing) under Work Order # 145079.

Elevated moisture was detected in the bottom portion of the cabinet and the wood facing located on the backside of the cabinet. 

Mold growth was observed on the laminated portion of the cabinet (back wall). Mold growth was also observed on the backside of the laminate and on the front of the plywood the laminate was adhered too. 

The visible mold growth was cleaned/disinfected upon discovery. Additional mold growth remains on the underside of the laminate.

Corrective Actions
January 7th, 2020

The mold impacted portions of the cabinet were removed on January 9. The water stained plywood shelf within the cabinet was stripped of its mold impacted laminated outer-shell (discarded), cleaned/disinfected and then dried out using fans. A mold inhibiting encapsulate was then coated onto the existing plywood. Once dry, a new laminated outer-shell was adhered.
