Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Black spots were observed on three sets of metal blinds (non-porous) in the cafeteria area. These black spots were also visible on the skylight above.
The spots did not appear to be visible mold growth. A moldy/musty odor was not present.
The custodian reported that the skylight above does leak.
The black spots are believed to be dirt/grime that was located on the roof and transferred to the blinds when the leak occurred.
The Office of Building Maintenance was contacted and reported that the skylight repair work is scheduled for the Summer of 2025.
The Office of the Environment and Custodian attempted to wipe the material clean. The staining can be wiped clean with a rag and cleaning solution, however a light staining was visible after the cleaning.
The school inquired about replacement blinds.
The Office of the Environment recommends cleaning the blinds for the time being as leaks may persist until Building Maintenance repairs the skylight. The blinds can be replaced following Building Maintenance's repair.
Alternatively, the blinds could be removed for the time being to avoid the need for cleaning. However, this may have security implications should a lock-down be needed. If Administration decides to remove the blinds until the repairs are made, this should be discussed with the Office of Safety and Security.
