Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

General Information
Custodial staff reported that the unit ventilator in Room 82 experienced a water leak on September 3, 2026. The leak was reportedly attributed to a clogged drain line associated with the unit ventilator.
The drain line was subsequently cleared and replaced. Following completion of the repair, custodial staff cleaned and extracted the affected carpet twice.
September 9, 2026 Assessment
During the assessment, temperature, relative humidity, and carbon dioxide levels were measured and found to be within the acceptable ranges.
The carpet was assessed using a moisture sensor. No elevated moisture readings were identified in the areas evaluated at the time of the assessment.
No visible evidence of mold growth was identified. No detectable mold-like or musty odors were noted at the time of the assessment.
A visible penetration was observed near the unit ventilator. An exterior inspection determined that the grille associated with the unit ventilator had become detached from the exterior wall. The detached grille and resulting opening represent a potential pathway for moisture intrusion.
