Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The Office of the Environment visited the site and spoke with onsite staff. Custodial staff stated that the vending machines were previously located in front of the bulletin board of concern and were moved to a new location during the summer. They also indicated some paper items were also impacted by suspect mold growth, however they had already been discarded.
No visible leaks were observed around the area of concern or above the ceiling tile.
Moisture was measured in the drywall around the bulletin board using a moisture meter. No elevated moisture in the drywall was observed.
Less than one square foot of suspect mold was observed on the bulletin board.
The Office of the Environment attempted to clean the bulletin board but, residual staining remained.

Due to the porous nature of the bulletin board, Building Maintenance (Carpentry) removed the bulletin board and discarded it.
The Office of the Environment observed that the bulletin board had been removed and no visible mold or obvious water damage was observed on the drywall previously behind the bulletin board.
No obvious visual indications of moisture was observed behind the new location of the aforementioned vending machine.
A thermal infrared camera was used to observe the drywall behind the current vending machine location and no indications of thermal anomalies indicative of moisture intrusion were observed.
Building Maintenance (Carpentry) removed and discarded the cork bulletin board. Drywall behind the bulletin board was painted to match surrounding area.
