Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The room contains counters along the wall (right side when entering). The sink/cabinet in question forms a "peninsula," extending from the main cabinetry along the wall. Additionally, another standalone cabinet (not attached to the main cabinets) that appeared to have been installed at a later date was located at the end of the peninsula.
Elevated moisture was detected in the baseboard in various locations around the "peninsula." Visible water damage was observed on the standalone cabinet. Elevated moisture was not detected in the main cabinetry along the wall.
A leak had been occurring over time due to the traps (located under the sink) being rusted.
The cove base was removed in several locations. Staining and delamination was observed on the particle board baseplates. Approximately one-two square feet of mold growth was observed on the backside of the cove base. A musty odor was observed upon removal of the cove base.
No visible mold growth or elevated moisture was detected within the cabinets.
The cove base was re-adhered after the assessment until Building Maintenance could correct the plumbing issue. Following the replacement of the traps, it was recommended to remove all impacted portions of the cabinetry. Impacted portions of the cabinetry that could not be removed were to be cleaned and encapsualted.
Building Maintenance (Plumbing) replaced the corroded traps and replaced the faucet.
Building Maintenance (Carpentry) removed and replaced impacted bases/cove base. The bottom of cabinet (inside) was replaced with a moisture resistant material. A new counter top was also installed. The stand-alone cabinet that appeared to have been installed at a later date was removed.
