Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The general Safety and Security area (Division of Operations) as well as the Director's Office were assessed.
Director's Office
The temperature (71.7 degrees Fahrenheit (F)), relative humidity (34.2%), carbon dioxide (466 parts per million (ppm), and carbon monoxide (0.0 ppm) were acceptable.
No moldy/musty odors were identified in the space.
Mold growth was not identified on surfaces.
The space was scanned for thermal anomalies indicative of moisture intrusion. Areas above the exterior windows showed thermal anomalies. However, when tested with a moisture meter, no elevated moisture was detected. The thermal anomalies are likely related to the metal around the windows being more thermal conductive than drywall, ceiling tile, etc. (colder outside) and does not represent a concern based on moisture testing.
The carpet was probed with a moisture meter. No elevated moisture was detected in the carpet.
The lower perimeter of drywall was probed with a moisture meter in accessible areas. No elevated moisture was detected.
Areas above the drop ceiling were observed. No concerns such as odors, visible mold, or moisture intrusion were observed.
The supply diffusers in the space were qualitatively assessed and were delivering air to the space.
Mild dust was observed on the supply diffusers.
Moderate to heavy dust was observed on horizontal surfaces to include on the top of cabinets, within cabinets, on the blinds, and in the crevices of the exterior windows.
Safety and Security Area
The temperature (72.2 degrees F), relative humidity (32.8%), carbon dioxide (451 parts per million (ppm), and carbon monoxide (0.0 ppm) were acceptable.
No moldy/musty odors were identified in the space.

Moderate to heavy dust was observed on horizontal surfaces; most notably on the top of cabinets and the top rails of the cubicle walls. Various picture frames, name tags, and other items were located on the tops of cabinets making these areas less accessible for custodial cleaning.
The Air Handling Unit (AHU-1) that serves the space was assessed:
- The outside air dampers were open.
- The filters were in place and in good condition.
- The cleanliness of the coils was acceptable.
- An access panel was opened in the return duct to observe the sound liner. No mold growth was observed on the sound liner.
Custodial Services is to dust the supply diffusers and horizontal surfaces. If needed, the Custodian is to work with individual offices to coordinate staff to move their personal items. Once complete, regular dusting per the Custodial Services cleaning schedule is to occur on a routine basis.
