Concern
HVAC leak occurred in back wing impacting the carpet in several rooms. The event occurred on September 27, 2022.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

September 27, 2022

It was reported that the HVAC coils burst almost simultaneously in Rooms 32, 35, 36, 37, 38, 39, and 41. The coils are centrally located within the rooms. Water came down primarily impacting carpets in each room. Portions of the fiberglass duct insulation (closets to the Variable Air Volume (VAV) Boxes) were also impacted in each of the effected rooms.

The coils were isolated and no longer significantly leaking (still dripping) when the Office of the Environment arrived.

Custodial Services had already begun extracting carpets when the Office of the Environment arrived. 

Fans were placed on the carpet to facilitate drying.

The wet/damaged fiberglass insulation will be replaced by Building Maintenance. 

As a precaution, a moisture meter was used on the casework in each room. With the exception of Room 37, no elevated moisture was detected.

In Room 37, the cove base was removed from the casework, holes drilled (to facilitate drying), and a dehumidifier was placed. 

Some water had entered the storage closet of Room 37, getting under the file cabinet and wetting some boxes. The file cabinet was placed on blocks to dry underneath. The boxes were moved into the classroom (with fans) to dry. The boxes can remain or be discarded at the discretion of the teacher.

September 29, 2022

The carpet and casework (Room 37) were assessed with a moisture meter and found to be dry.

An odor associated with wet fiberglass was noted. Wet fiberglass was found in a nearby trashcan and was discarded. 

The response actions above are conducted to prevent microbial growth.

Building Maintenance reported that the leak occurred due to a faulty switch in the Roof Top Unit (RTU) that services these rooms. This allowed the unit to overcool causing the coils to freeze and subsequently burst.

Corrective Actions

The insulation replacement and repairs to the RTU are being completed by the Building Maintenance Department under Work Order #255868.
