Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

It was reported that the Portable was approximately 86 degrees Fahrenhiet on May 28, 2021.
A new thermostat had recently been installed.
The HVAC Technician visited the site on May 28, 2021 to assess and found that the HVAC unit was not running.
No relative humidity mold was observed on surfaces.
No mold odors were sensed.
No concerns were observed above the drop ceiling.
A small hole was observed in the floor. The two thresholds in the center of the portable were not long enough to meet, leaving a one-inch gap.
No concerns were noted with the exterior of the portable.
A data logger was left in-place on the date of the assessment. No temperature concerns were identified.
Relative humidity was above ASHRAE recommended levels at times, however this likely due to the thermostat's fan being in the "on" position. HCPSS thermostats are typically not set to this position, however they are at this time due to COVID-19 (provides additional outside air).
No concerns were identified with carbon dioxide in the space (less than background + 700 parts per million (ppm).
The health concern noted could have been associated with the high temperatures noted on May 28, 2021.
Building Maintenance performed maintenance on the HVAC equipment and the unit is now functioning.
A work order was created for building maintenance to seal the small gap between the thresholds.
