Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The was school was primarily unoccupied due to closures associated with COVID-19.
Temperature and relative humidity were acceptable.
Mold growth was not observed on surfaces or the carpet.
Odors indicative of mold growth were not identified.
No potential IEQ concerns were identified above the drop-ceiling.
The HVAC system was not operational during the assessment due to the school closure.
A slight carpet odor was detected, however this odor is typical of a carpeted classroom. The odor was likely exacerbated as the ventilation system was not operational.
Other rooms with carpet were entered for comparison. The odor in Room 111 was similar to other carpeted classrooms within the building.
The unit ventilator within the room was assessed. No concerns with the unit were identified.
The unit's outdoor air intake (outside of the building) was observed. No concerns were identified in the vicinity of the air intake.
A thermal infrared camera was used to identify thermal differentials indicative of moisture intrusion. No significant thermal differentials were identified within the room.
