Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Fungal growth (not mold) was observed growing between floor tiles near the back door of the portable.
The floor tiles were removed easily (an indicator of moisture below). Elevated moisture was detected within the wood substrate below the floor tiles.
No elevated moisture was detected in the surrounding drywall.
It was reported that the portable was just moved from another facility.
The caulking and weather stripping around the portable appeared new and in good condition. It was reported that Building Services (Carpentry) re-caulked/weather-proofed the portable after it was moved.
The moisture intrusion at the door may have been caused by the move itself or there may have been an issue with the building envelope that was corrected by Building Services after the portable was removed.
The fungal growth was physically removed. Building Services (Carpentry) will remove the impacted sub-floor. While removing the sub-floor, Building Services will attempt to identify a potential source of the moisture intrusion and correct as necessary.
