Concern
A leak occurred in the 8th Grade Penthouse and was discovered the morning of April 2, 2019.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

April 2, 2019

The leak originated from Heating Ventilation and Air Conditioning (HVAC) piping in the 8th Grade Penthouse and had been stopped by the time the Office of the Environment arrived.

The Office of the Environment delineated the leak and found the impact to be in the penthouse, 2nd Floor Girls Bathroom, and the 1st Floor main  hallway (below the 2nd Floor Girls Bathroom).

Materials impacted included drywall and fiberglass insulation behind drywall (penthouse); drywall ceiling and decking insulation (2nd Floor Girls Bathroom); ceiling tiles, duct insulation, and decking insulation (2nd Floor Main Hall, next to Girls Bathroom), ceiling tiles and insulation (1st Floor, main hall, outside of Girls Bathroom).

Penthouse - Holes were drilled into the impacted drywall and fans were placed to force air into the wall cavity to facilitate drying of the drywall and insulation behind the drywall.

2nd Floor Girls Bathroom - The impacted drywall ceiling was removed by Building Services (Carpentry). The insulation on the decking above the drywall ceiling was peeled away and fans were placed to force air into the ceiling cavity.

2nd Floor Main Hall - The duct insulation and roof deck insulation  will be replaced by Building Services (HVAC). The ceiling tiles were discarded.

1st Floor Main Hall - The ceiling tiles and wet insulation (minimal) were discarded.

April 3, 2019

Penthouse - The penthouse drywall was not yet dry. Additional fans were placed in the penthouse to speed up the drying process.

2nd Floor Girls Bathroom - No elevated moisture was detected in the impacted areas. The Office of the Environment contacted Building Services so they could begin to reinstall the decking insulation and drywall ceiling.

April 5, 2019

Penthouse - No elevated moisture was detected in the drywall in the penthouse, however the thermal imagery camera showed a small thermal anomaly indicative of moisture. The fans will remain in the penthouse to facilitate drying over the weekend as a precaution.

The response actions noted above are intended to prevent microbiological growth. No further action is required as it relates to microbial growth.
