Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Administration
No elevated moisture was detected in the carpet.
The ceiling-mounted Fan Coil Units (FCUs) were inspected and were acceptable.
No elevated moisture was detected in the drywall.
No thermal anomalies indicative of moisture intrusion were detected.
The volume of outside air being delivered to the space was above the designed air flow. These readings are acceptable.
The area above the drop-ceiling was inspected and was acceptable. The area above the drop-ceiling contains fiberglass insulation on the roof deck (expected).
Housekeeping was acceptable.
Work Room
No elevated moisture was detected in the drywall.
No thermal anomalies indicative of moisture intrusion were detected.
The volume of outside air being delivered to the space was above the designed air flow. These readings are acceptable.
The area above the drop-ceiling was inspected and was acceptable.
A stained ceiling tile was observed. No mold growth was observed on or above the ceiling tile.
Minor dust deposits (possible chalk) were observed on a bookshelf and on the underside of the work table. The dust was cleaned upon discovery.
The crawlspace hatch was inspected. The hatch was sealed with caulk.
HVAC (OAHU-6)
One filter was installed backwards (corrected while onsite).
Minor particulate debris was observed in the unit (prior to filters).
The outdoor air dampers were open.
The coils and drain pan were acceptable.
Crawlspace
The crawl space is in good condition with plastic sheeting on the ground and sealed at the walls. Additionally, penetrations have been sealed with spray foam. There does not appear to be a significant nexus between the crawl space and the occupied potions of the building.
The particulate was cleaned out of OAHU-6.
If a work order does not already exist for the stained ceiling tile in the work room, Administration is to have a work order submitted to correct the leak. The stained ceiling tile can be changed once the issue has been corrected. If mold growth is observed on the ceiling tile before the issue is corrected, the ceiling tile is to be discarded and replaced.
