Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Mold growth (approximately 2 square inches) was observed on a wall.
It was reported that a bookshelf was formerly placed against the wall but had been moved during summer cleaning.
The bookshelf was assessed and mold growth was also observed on the backside.
The wall (drywall) was assessed with a moisture meter. Elevated moisture was detected.
An access panel was cut to observe the backside of the drywall. Mold growth was observed on the backside of the drywall as well.
Building Services (Roofing and Plumbing) was to assess the potential cause of he leak.
The mold growth on the wall/bookshelf was cleaned/disinfected upon discovery.
Building Services (Plumbing and Roofing) cut a hole into the chase to determine the cause of the leak. The cause of the leak could not be identified at the time of assessment.
Building Services (Carpentry and Roofing) visited the site after significant precipitation had occurred. No active leaks were identified. The impacted drywall was removed and replaced.
As a definitive cause could not be identified, an access panel was left in place to further assess behind the wall cavity in the future, if necessary.
