Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The space is finished with block wall, floor tile, and a drop ceiling. Several laboratory islands (for experiments) are located throughout the space.
A moldy/musty odor was sensed when entering the space. Another space within the same ventilation zone was also checked, however this space did not have an odor.
Each sink in the space was assessed for active leaks by running the water. No active leaks were identifed.
The kickplates for the various laboratory islands/casework was assessed. No elevated moisture was identified.
The floor drain near the eye wash had water in the trap.
The room consists of numerous plants with soil. Each plant was assessed for odors potentially emanating from the soil. One plant in particular had a very pungent odor. This plant was reported to have recently been added to the room.
The plant was removed from the room as a test and the odor dissipated. The staff member will have the plant removed permanently.
