Concern
When I am at FRES, I notice that my eyes and face are extremely red. I also get a headache when I am here and nose congestion. I am not sure if there is a presence of mold but I only notice these issues when I am at FRES.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date(s) of Assessment: January 20, 2026 

The space consists of carpet, drywall, casework, and a drop ceiling. 

The temperature (68.3 degrees Fahrenheit (F)), carbon dioxide (888 parts per million (ppm)), and carbon monoxide (0.0 ppm) were acceptable. The relative humidity (13.1%) would be considered low, however this is not unusual during the winter. HCPSS does not humidify via the Heating Ventilation and Air Conditioning (HVAC) system as it is not the industry standard for HVAC design for this region and it can introduce additional clinical environmental concerns (ex: humidifier fever).

The space was screened with a Photo-Ionization Detector (PID). No volatile organic compounds were detected. (0.0 ppm). 

No malodors were identified while in the space.

No mold growth or excessive dust was observed on surfaces.

Various surfaces were assessed for general cleanliness and/or mold growth. No visible mold growth was observed and the general cleanliness appeared acceptable. 

The space was screened with a thermal imaging camera. No thermal anomalies indicative of moisture intrusion were identifed. 

The drywall and kickplates associated with cabinetry were probed with a moisture meter. No elevated moisture was detected.

The carpet was probed with a moisture meter. No elevated moisture was detected. 

The carpet was screened with an ultra-violet light in an attempt to identify anomalies (spills, vomit, etc.). No anomalies were observed.  

The supply diffusers were assessed. The supply diffusers were delivering air to the space. No excess dust or suspect mold was observed on the supply diffusers or within visible portions of the flexible duct that supplies conditioned air to the space. 

A data logger was placed on January 20 and picked up on February 18 to measure Carbon dioxide. Carbon dioxide is an indicator if sufficient outside air is being delivered to the space. No elevated carbon dioxide readings were identifed, indicating sufficient outdoor air is being delivered to the space. 

The air handlining unit that serves the space was assessed:

  • The filters were in place and in good condition.
  • Heavy dust deposits were observed on the coils (energy management issue).  
  • The fiberglass sound liner is deteriorating and needs to be replaced. 
  • Housekeeping in the mechanical room was considered poor. 
Corrective Actions
April 1st, 2026

A work order was created to perform miscellaneous cleaning items in and checks in the HVAC unit that serves the classroom. This included, improving housekeeping in the mechanical room, cleaning the coils and condensation pan, replacing the sound liner, and verifying the outside air dampers are functioning properly. The work was completed April, 8 2026 with the exception of replacing the sound liner, which a contractor will perform.
