Air quality testing & resolution requested for rooms and issues listed below:
Room 102 and 226 - Black dust coming out of vents.
Room 200 - Black dust coming out of vents and a musty smell (worse in the first few weeks of the school year).
Room 220 - Black dust coming out of vents, Additionally, a severe leak occurred at the beginning of the year; water sat on carpet for weeks even with the trashcans. Mold check requested, but does not appear to have been done. When the ceiling tiles were removed, staff member had problems with dizziness/headaches when they were in the room that week.
3rd Floor Science Preparation Room (across from 311) - Discolored ceiling tiles and two of the 3 tiles that need to be replaced contain a sprinkler head and electronic box that need a special work order.
Room 106 - Discolored ceiling tiles, Air quality issues, and stained dirty carpeting.
Room 223 - Discolored ceiling tiles and black dust coming out of vents.
Room 320 - Dirty vent.
Room 206 - Water leaking, Discolored ceiling tiles, Air quality issues, Environmental Issues, Other request an air quality test,concerned about what is coming out of the ceiling. flies and have not had window open.

IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date(s) of Assessment: November 1 and 6, 2019

A simplified walkthrough was conducted due to the various concerns throughout the building. The Roof Top Units (RTU) were inspected on November 1, 2019 as part of the Office of the Environment's walkthrough process. No significant concerns were noted during the assessment of the RTUs.

Room 102

Minor dust deposits were observed on the supply diffusers.

One stained ceiling tile was observed.

Room 106

Stains were observed on the carpet.

No elevated moisture was detected  in the carpet.

Minor dust deposits were observed on the supply diffusers.

Room 200

The filters with the rooms unit ventilator were overloaded, degrading, and outdated.

An outstanding odor was not detected.

The odor sensed at the beginning of the year may have been a nuisance carpet odor. Carpet odors tend to build up over the summer due to doors being closed and differing HVAC operating schedules. Carpet odors are a nuisance and are not considered a health and safety concern.

Room 206

Temperature (73.9 degrees Fahrenheit), relative humidity (28.8%), carbon dioxide (800 parts per million (ppm), and carbon monoxide (0.0 ppm) readings were acceptable.

The unit ventilator was operational one-hour after school had dismissed.

An odor was sensed while in the space. A deceased mouse was found within the ceiling and may have been the cause of the odor.

The room contains a microwave.

The filters within the unit ventilator were overloaded, degrading, and outdated.

Room 220

No moisture was detected in the carpet.

No thermal anomalies indicative of moisture intrusion were identified.

No odors indicative of mold growth were observed.

No mold growth was observed.

A microwave and curtains were observed within the space.

The rooms unit ventilator could not be accessed, however based on the findings within the other spaces, the filters within the unit likely need to be replaced.

Minor water stains (dry) were observed on fiberglass pipe insulation above the drop ceiling. No mold growth or odors were observed/sensed above the drop-ceiling.

Room 223

Based on the findings with the other unit ventilators, the rooms unit ventilator was not assessed. The filters (school-wide) need to be replaced.

Stained ceiling tiles should be reported to Custodial Services.

Room 226

The unit ventilators filters had been recently changed. The cause of the black dust was likely the old filters.

3rd Floor Science Prep

Minor water staining was observed on ceiling tiles. No mold growth was observed.

Room 307

Minor staining was observed on the carpets.

No elevated moisture was detected in the carpet.

Two stained ceiling tiles were observed. 

No mold growth was observed.

No odors were sensed. 

Room 320

The filters within the unit ventilator are overloaded, degrading, and outdated.

Corrective Actions
November 12th, 2019

Custodial Services is to clean/wipe-down the supply diffuser vents in Rooms 102 and 106.

The cause of the black dust on unit ventilators is due to the overloaded, degraded, and outdated filters in the unit ventilators The entire school is due for a filter change. Building Services (HVAC) will begin changing filters on November 12, 2019.

Custodial Services is to place a work order for the stained ceiling tile(s) in Room 102, 223, 307, and 3rd Floor Science Prep. If a work order has already been placed, another work order does not need to be created.

November 12th, 2019

Administration is to determine the need for the curtains in room 220. If they are to remain, the individual who brought them in is responsible for cleaning them on a regular basis and applying fire retardant if applicable.

Administration is to determine the need for a microwave in Room 206 and 220. The Office of the Environment recommends the microwaves be removed if not needed for an educational purpose (saves energy).

A work order was placed for Building Services (HVAC) to assess and address the water stained pipe insulation in Room 220.

Unit ventiltator timing is to be assessed by Building Serviecs.

The mouse was removed from the ceiling and discarded upon discovery.

November 13th, 2019

Building Services indicated that the carpet has been in place since the building was constructed. Replacement of the carpet is on the deferred maintenance list.
