Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

General Information
Air Handling Unit 2 (AHU-2), which services the area of concern, had several issues, including a faulty compressor fan and issues with two compressor circuits. During the repair period, portable air-conditioning units were placed within the space to provide temporary cooling. Due to the HVAC issues, several staff members in the area reported concerns regarding mold and mildew odors. The issues associated with AHU-2 were corrected prior to the date of assessment.
Rooms 101–106 and Associated Workspace
The rooms were generally found to consist of a combination of vinyl floor tile and carpeted flooring, concrete masonry unit (CMU) and drywall walls, and suspended ceiling tiles.
Temperature, relative humidity, and carbon dioxide readings were collected throughout the areas assessed. All readings were found to be within acceptable ranges at the time of the assessment.
Carpeted areas were assessed using a moisture meter. No elevated moisture levels were identified within the carpets.
Walls and ceilings were assessed using a thermal infrared camera. No thermal anomalies that could be indicative of moisture intrusion were observed.
Drywall walls, wooden shelving, and cabinets were assessed using a moisture meter. No elevated moisture levels were identified.
No visible evidence of mold growth or mold-like odors was observed within the assessed spaces.
Faucets were operated and evaluated for leaks. No visible evidence of leaking was identified at the time of the assessment.
The plenum spaces (areas above the ceiling tiles) were evaluated in each room. No evidence of water intrusion or visible mold growth was identified.
Visible evidence of rodent feces was identified within the plenum space.
General Indoor Environmental Quality Concerns
The following general indoor environmental quality (IEQ) concerns were identified during the assessment:
Stuffed animals, fabric materials, personal furniture, and artificial plants.
These items can accumulate dust and may harbor allergens if they are not routinely cleaned and properly maintained.
Non-HCPSS-issued disinfectants
Disinfectant products not issued or approved by HCPSS were observed within the space. These products should not be used, as certain disinfectants and cleaning products may release airborne irritants when applied or used improperly.
Integrated Pest Management (IPM) to evaluate and install traps in the area.
School administration informed about the presence of stuffed animals, personal items, artificial plants, and Non-HCPSS issued disinfectants.
