IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: June 15, 2026

The office consisted of concrete masonry unit (CMU) walls, carpeted flooring, and drop ceiling tiles.

Carpet was evaluated for elevated moisture using a moisture sensor. No elevated moisture was identified.

A nuisance carpet odor was identified within the space.

The Office of the Environment is not aware of any health or safety concerns associated with carpet odors.

No return was observed within the space. Based on historical drawings, the office space was originally used as storage space.

Building Maintenance (HVAC) identified that the temperature control for the office space is located in the adjacent classroom.

Based on the above and the fact that the office space was originally used as storage and location of temperature control, the space could not be receiving sufficient air flow to dilute nuisance odors and allergens.

Corrective Actions
July 8th, 2026

Building maintenance (HVAC) is to place a return grille in the office space to assist with air flow.
