IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date(s) of Assessment: December 26, 2024 & January 3, 2025

December 26, 2024

A moisture readings were collected on the carpet. No elevated moisture was detected. 

A moisture meter was used to collect moisture readings from the drywall walls. No elevated moisture was detected. 

No stained ceiling tiles were observed. 

No obvious indications of moisture intrusion were observed. 

No obvious indications of moisture intrusion, leaks, or mold were observed above the ceiling tile. 

Visible dust was observed on the bookshelf and items on the book shelf. 

Several decorative items that appeared to have been brought in by staff had moderate to heavy accumulations of dust on them. 

Artificial plants were observed. 

Fabric items such as stuffed animals were also observed, presumably brought in by staff. 

January 3, 2025

Roof Top Unit-8, which serves the area of concern was assessed: 

  • The filters were observed in good condition.
  • Minor filter bypass was observed. 
  • Coils were observed in good condition. 
  • The condensate drainage pipe was observed to be disconnected from the unit. 
  • The relief dampener was observed to be damaged. 

Conclusion

The primary allergen observed in the work space that could be contributing to the concern was household dust, particularly on materials that were brought in by staff. The regular cleaning of items brought in by staff are the responsibility of the staff member who brought them in.  Additionally, the presence of some of these items hinders Custodial Services ability to properly dust.

The filter bypass within the HVAC unit may also play a role in the concern allowing unfiltered outdoor air in to the space. 

Corrective Actions
January 2nd, 2025

Artificial plants are not permitted as they collect dust and are difficult to clean. The staff member who brought them in is to remove them. 

January 2nd, 2025

The regular cleaning of items brought in by staff (decorative items, collectibles, etc.) are the responsibility of the staff member that brought them in. The Office of the Environment recommends removing any unnecessary items.

Additionally, any items that are to remain should be removed occasionally to allow the Custodial Services team to dust the areas where the items are located. The timing/frequency should be coordinated with your onsite Custodian. 

January 3rd, 2025

A work order was placed for Building Maintenance (HVAC) to assess/correct the minor filter bypass observed.

Although not likely related to the  concern, the work order also include:

  • repair the condensate drain pipe so that it discharges to a roof drain. 
  • repair the relief damper.
