Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Temperature, relative humidity, and carbon dioxide readings were collected within the space and were found to be within acceptable ranges.
The walls and ceilings were assessed using a thermal infrared camera. No thermal anomalies indicative of water intrusion were observed.
The carpet was assessed using a moisture meter, and no elevated moisture levels were identified.
No visible signs of water intrusion or mold growth were observed above or below the ceiling tiles at the time of the assessment.
No malodors were observed at the time of the assessment.
Based on discussions with school administration, an instrument had been stored in the conference room. Staff reported that the odor was no longer present after the instrument was removed from the area. The instrument was not present at the time of the assessment.
The Office of the Environment confirmed with school staff that the odor remained absent as of September 24, 2026.
Based on the observations and information provided, the odor appears to have been associated with the instrument previously stored in the area. No environmental conditions indicative of an ongoing moisture or mold-related odor source were identified at the time of the assessment.
