Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Temperature (71.5 degrees Fahrenheit), carbon dioxide (755 parts per million (ppm), and carbon monoxide (0.0 ppm) readings were acceptable.
Relative humidity (28.2%) would be considered low, however this is expected for the fall/winter. HCPSS does not introduce humidity as it can lead to clinical environmental illness' such as humidifier fever.
No odors were present during the assessment.
No mold growth was identified.
No elevated moisture was detected in the carpet or drywall.
Minor water staining was observed above the drop-ceiling on drywall. The staining appeared old. No elevated moisture was detected in the vicinity of the staining. No apparent leaks were observed. This was likely from a historic issue or a "one-time" event. No further action appears warranted in relation to the minor water staining.
Food items were observed in the desk. The food items were not stored in a plastic container.
Sprays such as "Lysol" and "Barbasol" were observed. Additionally, various creams and lotions were observed.
Horizontal surfaces such as desks and the chalk-try did not appear to have an excessive amount of dust on them.
Dust and debris (possible chalk-dust) was observed on the carpet.
The Air Handling Unit (AHU-5) that services the space was inspected. No concerns were identified with the AHU's filters, coils, or outside air dampers.
The individual noted that the vent filters have not been changed. The filters reside within the AHU (within the penthouse on the roof). The filters are up-to-date.
A request was made for Custodial Services to perform a precautionary carpet cleaning. The work is scheduled to be completed on December 9, 2019.
Food items are to be stored in sealed plastic containers to prevent pests. Administration is to ensure the food items are removed or stored properly.
Various chemicals and cosmetic products are located within the classroom. The individual can consider removing these products as they may be influencing the allergy symptoms reported.
