Concern
A mechanical pipe component serving an air handling unit within the penthouse failed and a caused a leak that infiltrated to occupied areas below.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date(s) of Assessment:  November 19 and 23, 2020.

The Office of Environment was informed of the incident by Building Services.  Upon arrival gross water was already extracted by Custodial Services and wet ceiling tiles removed.  The Office of Environment performed an assessment for areas and materials impacted and determined the need for a Board approved restoration contractor to assist with drying.

The areas and materials impacted:

Penthouse:  Minor amount of drywall (bottom) to a room containing hot water heaters.

Server Room / Room 225:  Bottom of drywall walls.  Portions of the wall consisted of two layers of drywall.  The wall was insulated with fiberglass.

Teacher's Lounge:  No items needing attention.

Media Center:  Carpeting and bookcases along the wall shared with Room 225.  Bottom shelves were removed for assessment and drying of drywall.

Room 143:  Base of drywall at second riser level and a section of carpeting and HVAC duct insulation.

Impacted items were either removed and replaced or dried using blowers and dehumidifiers. 

On November 23, 2020, the Office of Environment and restoration contractor determined impacted items to remain were dry.

The response actions above are intended to prevent microbial growth.

Corrective Actions
December 8th, 2020

Building Services did the following as a result of the incident:

Repaired the cause and inspected all air handling for similar issues. 

Inspected for and sealed possible voids from the penthouse to the below occupied area.

Evaluated floor drains within the penthouse.

Made repairs to impacted building materials.
