IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: December 21, 2023

The Office of the Environment was informed that a fire extinguisher was discharged inside the portable on December 21, 2023. The Office of the Environment arrived onsite and interviewed the teacher and assistant principal to ascertain how the fire extinguisher was discharged. According to the teacher, students were lining up to leave the portable and two of the students began shoving one another. One of the students was shoved into the fire extinguisher which fell and discharged. 

The students and teacher evacuated the portable and proceeded to the nurse's office to be evaluated.

A board-approved remediation contractor cleaned the portable using a High Efficiency Particulate Air (HEPA) vacuum. After the initial vacuuming was completed, each surface was wiped with a cleaning agent/disinfectant. The surfaces were once again HEPA vacuumed after residue from wiping had dried. The remediation contractor also placed two HEPA air purifiers to operate overnight to reduce suspected particulate from the discharge event/cleaning activities.

The Office of the Environment arrived onsite prior to the contractor to verify that areas had been cleaned. The Office of the Environment requested that the remediation contractor re-clean a few areas and observed the remediation contractor clean the requested areas.
