Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

No visible mold was observed within the classroom or ductwork. No moldy, musty, or other malodors were identified.
No elevated moisture was identified within the carpet using a moisture sensor.
No elevated moisture was identified within the drywall walls using a moisture meter.
Moderate to heavy dust deposits were observed on musical instruments. Moderate to heavy dust deposits were observed on shelving units, metal file cabinets, and staff effects.
Two school-issued air purifiers were observed within the room against the walls. One of the purifiers was observed to be off and was turned on by the Office of the Environment.
The musical instruments are to be dusted. The Office of the Environment understands that the dusting of musical instruments in the responsibility of the music staff member in the space (not custodial).
Custodial is to vacuum the dust from shelving, file cabinets, and furniture.
The air purifiers were located within a foot of the walls, which restricted air flow clearance. Staff member was informed that providing air purifiers with three feet of clearance would likely increase the effectiveness of the unit.
