Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The rooms consisted of concrete masonry unit (CMU) and drywall walls, wooden bookshelves, carpeted flooring, and a drop ceiling with ceiling tiles.
Temperature, carbon dioxide, and carbon monoxide levels were within acceptable ranges. No detectable levels of volatile organic compounds (VOCs) were identified using a photoionization detector (PID).
No elevated moisture was detected in the carpet using a moisture sensor. Additionally, no elevated moisture was identified in the drywall or wooden bookshelf using a moisture meter.
No visible mold growth or evidence of water intrusion was observed above or below the drop ceiling tiles.
A variety of personal items were present throughout the space, and visible dust accumulation was observed on several surfaces.
Air Handling Unit 2 (AHU-2), which provides cooling and outside air to the areas of concern, was assessed as part of the investigation.
No apparent deficiencies were observed with AHU-2 at the time of the assessment.
Staff members and school administration was informed of deposits of dust present on personal items within the offices and recommended that staff work collaboratively with custodial services to establish a consistent and effective cleaning plan.
