Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Room 204
The room is finished with block walls with one column (drywall), floor tile, and a drop ceiling.
The temperature (71.2 degrees Fahrenheit (F)), relative humidity (50.8%), carbon dioxide (430 parts per million (ppm), carbon monoxide (0.0 ppm), and Volatile Organic Compound (VOC) concentrations (0.0 ppm) were acceptable.
No moldy/musty odors were identifed.
The room was screened with a thermal infrared camera. No thermal anomalies indicative of moisture intrusion were identifed.
The drywall column was probed with a moisture meter. No elevated moisture was detected.
No moisture intrusion concerns, odors, or visible evidence of a leak were identifed above the drop ceiling.
The supply diffusers were delivering air to the space.
Minor dust was observed on the metal supply diffusers.
The chalk tray contained excessive chalk dust.
The Air Handling Unit (AHU) that serves the space (AHU-1) was assessed:
- The outside air dampers were open.
- The filters were in place, in good condition (replaced March 2025), with no bypass observed.
Room 301
The room is finished with three block walls, one drywall wall, two columns (drywall), floor tile, and a drop ceiling. The room also contains casework with built in sink.
The temperature (71.0 degrees F), relative humidity (51.0%), carbon dioxide (398 ppm), carbon monoxide (0.0 ppm), and VOC concentrations (0.0 ppm) were acceptable.
No moldy/musty odors were identifed.
The room was screened with a thermal infrared camera. No thermal anomalies indicative of moisture intrusion were identifed.
The drywall wall and columns were probed with a moisture meter. No elevated moisture was detected.
No moisture intrusion concerns, odors, or visible evidence of a leak were identifed above the drop ceiling.
No elevated moisture was detected in the kickplates of the casework or in the shelving under the sink.
The supply diffusers were delivery air to the space.

No moldy/musty odors were identifed.

- No concerns were identified with the coils/drain pan.
The supply diffusers in all three rooms need to be cleaned. It may be that the frequency of supply diffuser cleaning in general needs to be increased.
The chalk tray should be vacuumed with a vacuum equipped with a High Efficiency Particulate Air (HEPA) filter.
