Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Staff members reported that the heating, ventilation, and air conditioning (HVAC) unit serving the gymnasium was not working properly.
Temperature, relative humidity, and carbon dioxide (CO₂) readings were collected during the assessment. Relative humidity was elevated at 85.8%.
No visible or olfactory indications of mold growth were identified within the space at the time of assessment.
A door sweep was observed to be missing, providing a potential pathway for moisture intrusion.
Air Handling Unit 9 (AHU-9) was operational at the time of the assessment.
AHU-9 was not operating normally, resulting in the unit circulating unconditioned outside air into the gymnasium. This condition contributed to the elevated relative humidity observed within the space.
AHU-9 was temporarily shut down to prevent unconditioned air from entering the space until a contractor could evaluate the operation of the unit.
The Office of School Planning and Construction (OSPC) contracted a design engineer to review the AHU-9's operation and revised the sequence of the unit to provide better dehumidification within the space.
