Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

While filling a fish tank (clean water), a staff member left the room for a period of time without turning off the water. The tank overflowed impacting the kick-plates of the of wooden cabinetry the tank was sitting on.
The cove base was removed to drill holes which assist with air flow to facilitate drying.
Suspect mold growth (approximately one to two-square feet) was observed on the kickplates. The kickplates were removed and replaced.
The side panel of the cabinet (closest to the prep room) was cleaned/disinfected as a precaution prior to re-adhering the cove base (could not be removed since structural).
Water also traveled into the adjoining Room 106, however no porous materials were impacted.
Standing water was addressed by Custodial Services.
Impacted wood surfaces that were not removed due to suspect mold growth were re-assessed for elevated moisture on August 22, 2023. No elevated moisture was detected.
