Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Investigation occurred approximately 7:00 a.m. and weather consisted of overcast and rain. The week experienced tropical like weather with rains due to a coastal storm.
An odor was not detected upon entry into the portable.
Mold indicative to excessive humid conditions was not observed on inspected surfaces.
The walls and ceiling did not show any signs of moisture concerns as determined with the use of a moisture meter and thermal imager.
The HVAC unit's outdoor air damper did not pop open when the fan was operating.
Indoor conditions were 74 degrees and 55% relative humidity.
The thermostat was properly set as for fan mode ("auto") and temperature set points (68 heat and 76 cool).
Work orders were reviewed from December 2019 to present. Work orders 131853 (9/2019) and 140120 (11/2019) reported thermal discomfort (81 degrees). HVAC personnel did not report any issues during those investigations as for system operation.
A work order was created for Building Maintenance (HVAC) to assess and correct the HVAC unit's outdoor air damper.
