Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Custodial staff reported visible staining on the carpet and ceiling; however, the affected areas had been cleaned prior to the Office of the Environment’s assessment.
The thermostat was observed to have improper settings, with the fan set to “On” rather than “Auto.”
Visible mold growth was identified on furniture within the portable classroom.
The mold growth is likely associated with the improper thermostat settings, which may have introduced unconditioned air into the space over an unknown period of time.
Exterior Vestibule
Custodial staff reported staining and suspected mold growth on the exterior vestibule ceiling. The ceiling had been cleaned prior to the Office of the Environment’s assessment.
The water staining and suspected mold growth are believed to have been caused by a clogged drain line associated with a unit ventilator located on the second floor directly above the exterior vestibule.
No thermal anomalies indicative of active water intrusion were identified in the exterior vestibule or within the adjacent interior areas of the building.
No visible mold growth was observed on the exterior vestibule ceiling or on interior surfaces adjacent to the exterior vestibule.
A moisture meter was used to evaluate the moisture content of building materials. No elevated moisture readings were observed at the time of the assessment.
The thermostat was changed to correct settings.
The Office of Environment and custodial staff cleaned and disinfected impacted furniture.
A drain line on the HVAC unit above the vestibule was unclogged.
