Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The walls within the area of concern were constructed out of drywall, concrete masonry unit (CMU) blocks, and bulkhead glazing system. The space was also finished with carpet floors and drop ceiling tile.
No elevated moisture was observed in the carpet via a moisture sensor.
No thermal anomalies were observed in the walls or ceiling via an infrared thermal camera.
No visible indications of leaks was observed except two stained ceiling tiles. Ceiling tiles were not observed to be actively wet at the time of the assessment.
No visible indications of mold were observed above or below the ceiling.
No elevated moisture was identified within the drywall using a moisture meter.
A nuisance carpet odor was observed in the space. No other malodors were observed. The Office of the Environment is not aware of any health or safety concerns associated with carpet-related odors.
The Office of the Environment recommended that the affected carpeted areas undergo sanitization and hot water extraction cleaning to custodial management.
This measure is intended to address the observed nuisance carpet odor and help prevent future occupant concerns that may be mistakenly attributed to indoor environmental quality (IEQ) conditions.
