IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: February 27, 2026

The Office of the Environment was informed by staff that water was observed dripping from the ceiling. 

First Floor Custodial Closet

No visible indications of water dripping from the ceiling was observed at the time of the assessment. 

Thermal anomalies indicating water intrusion were observed via an infrared camera. 

The Office of the Environment observed approximately eight-square feet of visible mold growth on the upper side of the drywall ceiling within the first floor custodial closet. 

The metal decking above the ceiling in the first floor custodial closet was observed to be rusted. This could potentially be caused by water intrusion originating from an upper floor.   

Second Floor Custodial Closet

The second floor custodial closet, which is located directly above the first floor custodial closet was assessed. 

The custodial closet was observed to have water stained floor tiles and portions of metal shelving in contact with the floor were rusted. 

The floor was observed to be noticeably sloped towards the general location where rust was observed on the decking below.

No visible mold growth was observed within the closet or above the ceiling. 

No evidence of water intrusion was observed above the ceiling in the second floor custodial closet. 

Building Maintenance (Plumbing) investigated the custodial slop sink in the second floor custodial closet with the Office of the Environment present to determine if the slop sink was the source of water intrusion. No indications of leaking were observed in conjunction with the second floor slop sink. 

Based on the testing and observations, the source of the mold growth may be due to the sloping of the custodial closet and potential penetrations at the block wall and floor. Incidental spillage from custodial activities likely migrates to the ceiling on the first floor. 

Corrective Actions
March 3rd, 2026

A board-approved restoration contractor to removed the impacted drywall ceiling. 

March 5th, 2026

Building Maintenance (Carpentry) to remove covebase from second floor custodial closet and apply sealant at joint between wall and floor to prevent water from traveling down. 

March 5th, 2026

New ceiling to be installed within the first floor custodial closet.
