Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The room is finished with floor tile, block walls, and a drop ceiling. The space also has built in casework. The Heating Ventilation and Air Conditioning (HVAC) system consists of a wall-mounted unit ventilator.
No odors were identifed during the assessment.
No elevated moisture was detected in the kickplates of built-in cabinetry/casework.
No odors or moisture intrusion concerns were identifed above the drop ceiling.
The space was screened with a thermal infrared camera. Thermal anomalies possibly indicative of moisture intrusion were identifed around the windows. No other thermal anomalies were identifed. No thermal anomalies were identifed above the drop ceiling, including above windows.
Flaking peeling paint indicative of moisture intrusion was observed near the windows.
No moldy/musty odors were identifed in the vicinity of the windows.
The unit ventilator cover was removed. No odors were identifed emanating from the unit.
No odors emanating from unit. Minor debris in unit.
Work order created for Building Maintenance to clean/check the unit ventilator. While cleaning the unit, the technician was able to locate a dead rodent (removed). The unit was cleaned/disinfected.
Building Maintenance/Office of Construction indicated that the are assessing the windows of the school (not just the room associated with this concern) to be included as a project in a future capital budget.
