Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The Heating Ventilation and Air Conditioning (HVAC) had already been assessed by Building Maintenance (HVAC) under work order #39880.
No visible or olfactory indications of mold were observed.
No indications of water intrusion were observed on block walls, ceiling tiles, or above the ceiling tiles.
No elevated moisture was identified within the carpet utilizing a moisture sensor.
The unit ventilator was observed to be operational at the time of the assessment.
The filter within the unit ventilator appeared to be in good condition.
No condensation on the pipes were observed within the unit ventilator.
Temperature (74.9F), relative humidity (39.1%), and carbon dioxide (472 ppm) were collected. The readings were found to be acceptable. The relative humidity at the time of the assessment was not conducive for mold growth.
A faint smell of carpet off-gassing was observed during the time of assessment (approximately 6:45 am). The odor experienced is a nuisance odor and does not represent a health and safety concern.
Based on the lack of conditions conducive to mold growth and lack of visible and olfactory evidence of fungal growth, no further actions are warranted at this time.
