Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Two soiled areas (each approximately the size of a quarter) were observed near the exterior door. The soiled areas were "hard" and were able to be scraped away from the carpet using hand pressure/fingernail.
The cove base in this location was peeling from the wall and pest activity was observed behind the cove base. Both are evidence of a moisture intrusion issue.
The nearby drywall was probed with a moisture meter. No elevated moisture was detected.
The carpet was probed with a moisture meter. No elevated moisture was detected, unless probing directly onto the soiled areas. Excessively soiled areas can give a false positive on the moisture meter due to the possible presence of salts (electrical conductivity).
Areas above the drop ceiling were assessed for potential moisture sources. No signs of leaking were observed above the drop ceiling.
The exterior door was assessed. No signs of "daylight" were observed and the condition of the exterior caulk was acceptable.
The adjoining Room 20 was assessed. The exterior block wall showed signs of bubbling paint, which is also indicative of a moisture intrusion issue.
An exterior roof drain is located outside of these areas.
Work order was created for Building Mainteiance to address the cause of the leak.
