Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Building Maintenance was called to the site to assess a "soft" floor. The assessment involved removing drywall in the area. Suspect mold growth was observed behind the drywall upon it's removal. Building Maintenance contacted the Office of the Environment upon discovery.
The Office of the Environment arrived onsite and confirmed that mold growth was on the backside of the drywall. The impacted drywall (as well as drywall beyond the area of impact) had already been removed and placed in trash bags for disposal.
Several studs were stained and appeared to be "rotting."
A slit was observed in the vapor barrier in the location where the drywall was removed. This may have caused condensation to migrate through causing the issue. Additionally, Building Maintenance noted that this portable had been recently moved. The moving process involved taking off the gutters. When the gutters were re-attached, the screws were not installed into the pre-existing holes, new holes were drilled. However, the pre-existing holes were not sealed.
The impacted drywall was removed on November 16, 2022. The impacted studs and sub-flooring will be replaced with work anticipated to be complete on November 18, 2022. Building Maintenance sealed the former gutter attachment penetration's on November 16, 2022. The slit in the vapor barrier will also be repaired prior to re-installing the drywall.
