Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Date of Assessment: September 17, 2026
Rooms 12 and 21
Temperature, relative humidity, and carbon dioxide readings were collected in the spaces. All readings were within acceptable ranges at the time of the assessment.
Carpet flooring was evaluated using a moisture meter, and no elevated moisture levels were detected.
Walls and ceilings were assessed using a thermal imaging camera. No thermal anomalies suggestive of active moisture intrusion were identified at the time of the assessment.
Faucets were operated for several minutes, and no leaks were observed.
Wood shelving, cabinetry, and drywall were evaluated using a moisture meter. No elevated moisture levels were detected.
Fan coil units (FCUs) were inspected and found to be operational. Filters appeared clean and were dated. Coils and drain pans appeared clean and free of visible debris.
No visible evidence of mold growth was identified above or below the ceiling grid.
No unusual odors were detected by the Office of the Environment or reported by staff members at the time of the assessment.
A staff member reported previously noticing an odor described as resembling “sour laundry” near the beginning of the school year. The staff member indicated that the odor had not been noticed recently.
On September 28, 2026, staff reported that no unusual odors had been observed in the room 12 and 21.
