Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The Office of the Environment was notified of an HVAC-related water leak in Room A15 on August 17, 2026.
The cause of the leak was reportedly due to a condensate pan overflow associated with Roof Top Unit 3A (RTU-3A).
During the subsequent assessment, elevated moisture levels were identified within a drywall wall assembly in the affected area. To facilitate access and promote drying of the affected wall assembly, a bulletin board was removed from the wall.
Following removal of the bulletin board, approximately 20 square feet of visible mold growth was identified on the drywall.
Based on the observed conditions, the mold growth is believed to be associated with a historical moisture intrusion event resulting from a previous leak.
Building Maintenance (HVAC) unclogged the clogged drain line associated with RTU-3A and the roof drain that the drain line feeds into.
A Board-approved restoration contractor built an enclosure around the area of impact. Additional mold was identified on back on removed drywall. Additional materials were removed until visible mold growth was no longer present.
