Concern
It was reported a contractor left a valve open when filling the Heating Ventilation and Air Conditioning (HVAC) system (summer project), causing a leak to occur. Moisture impacted drywall was identified.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: August 14 and 15, 2023

The Office of the Environment was contacted on August 14, 2023 to evaluate water damaged materials from a leak that was reported to have occurred on August 10, 2023, due to an open valve in room 196. 

During the assessment, moisture impacted drywall materials were identified within the vicinity of the leak. 

The Office of the Environment contacted a board-approved restoration contractor to mobilize to the site and remove the moisture impacted drywall as it had been wet for several days. 

Additional assessment also identified elevated moisture in the kick-plates of the casework. Upon removal of the cove base, mold growth was observed on the backside of the cove base/front side of the kickplates. 

The casework was removed  from the wall so that the drywall behind the casework could be further assessed. Elevated moisture was detected in the drywall. Additionally, mold growth was observed on the backside of the casework (bottom portion), once it was removed. 

The wall behind the casework adjoined a band room associated with Lake Elkhorn Middle School. Water had migrated to the adjoining Band Room (Room 196). Please see IEQ Report #CXLEMS20230002AF) regarding the response actions associated with Lake Elkhorn Middle. 

Corrective Actions
August 15th, 2023

The impacted kickplates, cove base, and drywall were removed by the board-approved restoration contractor. 

Building Maintenance was reminded to inform the Office of the Environment as soon as reasonably possible should a leak impact drywall or wooden materials.
