Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The Office of the Environment met with the occupant and the occupant indicates they spend time in several rooms. These rooms included 202, 210, 215, 219, and 220.
All of the spaces (expect room 202) are served by a dedicated Outside Air Unit (OAU-1) which provides outside air to the spaces for dilution. Room 202, is served by OAU-2. Each room also has an individual Fan Coil Unit (FCU) that re-circulates air in the space for temperature control.
Each room was assessed for general moisture concerns, malodors, and visible mold growth with no significant findings in any of the spaces.
The OAUs were assessed by the Office of the Environment on May 1, 2025 during a routine proactive Heating Ventilation and Air Conditioning assessment. The pre-filters in OAU-1 were found to be loaded with dust and needed to be changed. No other deficiencies were identifed with OAU-1 or 2.
The FCU's were assessed in each space. In general, moderate to high dust loading was observed on the fan wheels. The dust on the fan wheels has the potential to become airborne during operation of the unit and requires cleaning.
A work order was created during the Office of the Environments routine HVAC assessment on May 13 regarding the dust loading on the prefilter in OAU-1.
A work order was placed to Building Maintenance to clean the FCUs. Work was completed on June 5, 2025.
