Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Gym tumbling mats were stored in a boys locker room shower stalls. Plumbing related to a shower leaked to wet mats and/or create a humid environment.
Mold growth was found on at least eight 6' x 12' folding mats. The mats were removed form the locker room and placed in the gym. The mats were opened up and placed on wood wedges so not to rest against the gym floor. Fans and carpet blowers were placed to dry the mats.
A couple of mats were cut open to inspect the inner material which was more of a styrofoam and dry.
On July 23, 2021, a restoration company was utilized to clean and disinfect the mats.
The Office of Environment inspected the contractor's work and mats on July 23 and August 9, 2021. Work and mats were found to be acceptable.
The mats were disinfected. Building Services fixed the leak. The Office of Environment recommended that the mats not be stored in an area where they can be directly impacted by water (i.e. shower stalls).
The shower stall ceramic tile floor and walls were to be cleaned by custodial staff as they would normally clean to remove grime / staining and/or mold transfer. This included the tile grout.
