IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: September 22, 2021

A wall-mounted HVAC unit is located in the MDF Room. It was reported that the unit leaked the week of September 6th. It was also reported the unit has leaked in the past.

It was reported that Building Maintenance (HVAC) repaired the cause of the leak.

Two bookshelves (built-in) are located under the unit and were impacted by the leak. Mold growth was observed on the top-shelf of the casework and on several of the shelves beneath. Warping was also observed in several areas. 

It was reported that the books severely impacted have been disposed of. 

A hole was cut into the bookshelf to observed the back of the shelf and the wall (block) behind. No mold growth was noted behind the casework.

Corrective Actions
September 22nd, 2021

School staff reported that because the leak has occurred historically, they will store books in a another location. Since the shelving is no longer needed and is impacted by water damage and mold growth it will be removed.
